Nurse-Family Partnership empowers first-time moms to transform their lives and create better futures for themselves and their babies.
KCCDPHE serves Kit Carson, Cheyenne, Lincoln and Elbert counties.
Qualifications for the program:
- 1st time mother
- At or below 200% of federal poverty level (based solely on mothers income)
- Education level
