Priority Areas
This plan outlines three main priority areas for the next five years. These priorities aim to create a foundation for increased collaboration with community leaders and residents. The goal is to provide sustainable programs that enhance access to prevention and address the social determinants of health

 

1. Resource Guide

During the recent key leader meeting, this priority emerged as a central topic. By emphasizing this priority, we aim to enhance access to a wide array of resources available within the county and the surrounding region. These resources are designed to comprehensively address all social determinants of health, including economic stability, education, social and community context, health care access, and the built environment. This initiative is expected to foster a more holistic approach to health and well-being in our communities.
 

2. Chronic Disease

Chronic disease management has gained prominence through comprehensive data analysis, collaboration with influential leaders, and active community involvement. This initiative aims to enhance access to preventive education and resources across the county, specifically targeting individuals living with chronic conditions. By addressing existing gaps and reducing barriers to care, we aspire to improve health outcomes for our community members significantly. This proactive approach not only empowers individuals to take charge of their health but also fosters a supportive environment conducive to long-term well-being.

 

3. Transportation

Kit Carson County is situated in the eastern plains of Colorado. This region is characterized by vast landscapes, where the beauty of wide-open spaces is often offset by the logistical challenges its residents face. Access to essential transportation services is limited, significantly impacting the availability of basic needs and healthcare for the community. To navigate these challenges, there is an urgent need for creative collaboration among local community leaders, healthcare providers, and transportation officials.

This initiative will call for out-of-the-box thinking to develop innovative transportation solutions tailored to Kit Carson County’s unique needs. By working together, stakeholders can explore avenues such as rideshare programs, community shuttles, or partnerships with existing transportation networks.

Addressing this priority is pivotal for residents who currently struggle to reach medical facilities and obtain necessary supplies. Enhancing transportation access will not only alleviate these barriers but also foster greater community connectivity. Ultimately, this effort aims to empower the people of Kit Carson County, ensuring they can access vital healthcare services and meet their daily needs, thereby improving overall well-being and quality of life in this remote and resilient region.
